Welcome to decision4transition!
Older adults are at an increased risk of frequent transitions between care settings. These transitions are often associated with negative outcomes for the person concerned. Some of these care transitions are avoidable, and the phenomenon of avoidable transitions has received greater attention within the last two decades.
Some assessment tools to identify potentially avoidable care transitions were developed and were shown to be successful. However, existing instruments are limited to a specific care setting or population, and there is no such thing as a “golden standard” tool for a particular life situation.
Decision-makers, be it care professionals or older adults and their caregivers, may benefit from such existing tools. However, finding and choosing the right tool might be challenging.
We are glad to announce that we have launched, to our knowledge, the first database that embraces 29 various assessment tools. These tools are critically appraised and systematically organised to help you choose the right tool that fit your life situation. Our database is aimed to contribute to informed decision-making by supporting care professionals, older adults and their caregivers in finding the right solution.
